    • PAGET’S DISEASE (Osteitisdeformans)
      • a condition characterised by a progressive tendency for one or more bones to bend, get thickened and spongy (MCQ)
      • Tibia is the bone affected most commonly(MCQ)
      • Pathophysiology:
        1. a defect in the osteoclast functions, so that irregular bone resorption and increased bone turnover occurs.
        2. The bone is soft and vascularin the initial stages, but becomes dense and hard later.
      • The disease begins after 40 years of age. (MCQ)
      • Presenting complaints are dull pain, and bowing and thickening of the affected bone. X-rays show multiple confluent lytic areaswith interspread new bone formation.
      • Bone scan shows an increased uptake(MCQ)
      • Serum alkaline phosphatase is elevated. (MCQ)
      • Remember :
        1. High Serum Calcium with Normal Alkaline Phosphatase with normal uptake on bone scan is seen in Multiple myeloma (MCQ)
        2. Normal Serum Calcium with High Alkaline Phosphatase with increased uptake on bone scan is seen in Paget’s disease (MCQ)
      • Usual complications (MCQ)
        1. pathological fracture
        2. malignant change to secondary osteosarcoma
      • Treatment is by calcitonin or diphosphonate(MCQ)
