    1. It is lack of nasal resonance for words which are resonated in the nasal cavity, e.g. m, n, ng.
    2. It is due to blockage of the nose or nasopharynx
  2. Hypernasality (RhinolaliaAperta)
    1. It is seen when certain words whichhave little nasal resonance are resonated through nose.
    2. The defect is in failure of the nasopharynx to cut off from oropharynx or abnormal communication between the oral and nasal cavities

What is a Nasal Voice?

E.C., 7 years old with abnormal speech
7 year old is noted to have abnormal speech consisting of persistent audible nasal emissions and rustles. There is no evidence of clefting. Does he have occult submucous cleft palate? Velopharyngeal mislearning? Is the problem structural or physiological? Does he need surgery or therapy? What else can be done?
A.G. 3yo with VPI
Aileen G. age 3 years old underwent repair of a cleft palate in Phoenix, Arizona. She has persistent hypernasality, audible nasal emission, glottal stops, omissions, grimaces and low intelligibility. Original surgeon did not mobilize entire palate, instead, she used relaxing incisions which failed to yield sufficient length to achieve closure. Palate remains stiff and short with severe velopharyngeal insufficiency.

This boy is a case of Bilateral Cleft Palate and Lip. His complaints include hypernasality caused by Velopharyngeal insufficiency. The surgical plan is to correct the defect so that no air escapes in to the pharyngeal area while speaking – The procedure is the sphincter Pharyngoplasty.
Through the bifid uvula, an incision is made and the palatopharyngeus muscle and Superior constrictor msucle is identified and elevated. This is done on both sides and the flaps are fixed to the posterior pharyngeal wall just below the adenoid area. Care should be exercised to avoid the blood vessels in the region. By this procedure, a dynamic sphincter is created. On a suction test, the movement of the soft palate is seen, which an indicator that there is no air escaping is. This positive suction test is an indicator that the dynamic sphincter is working and the patient will have no nasal twang in his voice.
